This Consent and Privacy Notice explains how Reentry Care LLC (“Reentry Care,” “we,” “us,” or “our”) collects, uses, shares, and protects your information when you request health insurance enrollment assistance. It also explains how Reentry Care and its licensed insurance partners may contact you.

By submitting a Reentry Care form and checking the consent box, you agree to this Consent and Privacy Notice.

Request for Enrollment Assistance
You are requesting assistance understanding your health coverage options and enrolling in coverage if eligible.

Reentry Care is not a health insurance carrier and does not guarantee that you will qualify for financial assistance, Medicaid, Marketplace coverage, or any particular insurance policy. Licensed insurance agents and agencies are responsible for providing insurance advice, plan recommendations, and enrollment services.

Authorization to Share Your Information
You authorize Reentry Care to collect the information you provide and share it with one or more licensed insurance agents or insurance agencies that may be able to assist you.

The information shared may include your:

• Telephone number

• Email address

• Mailing address

• State and ZIP code

• Coverage needs

• Referral source

• Responses to questions on the Reentry Care form
   

The partner receiving your information may depend on your location, coverage needs, licensing requirements, partner availability, and other factors relevant to providing the assistance you requested.

Licensed insurance agents and agencies receiving your information are independent businesses responsible for their own services, communications, legal compliance, privacy practices, and information security.

Consent to Be Contacted
You authorize Reentry Care and the licensed insurance agents or agencies receiving your information to contact you about health coverage and enrollment assistance by:

• Telephone call

• Text message

• Email
   

Depending on the licensed insurance agent or agency contacting you, communications may be made using automated calling or texting technology, prerecorded messages, or artificial voice messages.

Message frequency may vary. Message and data rates may apply. Your consent is not a condition of purchasing any product or service.
You may opt out of text messages at any time by replying STOP. You may opt out of emails by using the unsubscribe link in the email, when available, or by contacting the sender.

You may withdraw your consent to be contacted by Reentry Care by emailing data@reentrycare.org. If your information has already been shared with a licensed insurance agent or agency, you may also need to contact that partner directly. Withdrawal of consent does not affect communications or disclosures that occurred before your request was received and processed.

Information We Collect
Information You Provide
We collect information that you voluntarily submit through our website, forms, communications, and referral process. This may include:

• Contact information, such as your name, telephone number, email address, and mailing address

• Location information, such as your city, state, and ZIP code

• Basic information used to determine what type of coverage assistance you may need

• Information about when you were released from incarceration

• Information about how you learned about Reentry Care

• Communication preferences

Please do not submit Social Security numbers, Medicare numbers, insurance member identification numbers, financial account information, medical records, or detailed medical information through our general website forms unless Reentry Care expressly requests that information through an appropriately secured process.

When you visit our website or submit a form, we or our service providers may automatically collect information such as:

• Referring website or referral source

Licensed Insurance Partners

These partners may contact you using the contact information you provided. Once your information has been shared, the receiving partner may maintain its own records as permitted or required by law.

Reentry Care does not control the independent privacy or security practices of licensed insurance agents and agencies. You may request information about the partner receiving your referral by contacting data@reentrycare.org.

Service Providers
We may share information with vendors that help us operate our services, including providers of:

• Customer relationship management systems

• Email and text-message communications

Referral Compensation
Reentry Care may receive compensation when it refers a request for enrollment assistance to a licensed insurance agent or agency. Compensation may be calculated based on the number of referrals provided or another agreed arrangement.

We do not sell personal information to data brokers or share it with third-party advertisers for unrelated advertising. Depending on where you live, applicable law may provide additional rights concerning the disclosure of personal information.

Cookies and Analytics
We may use cookies and similar technologies to operate our website, remember preferences, measure website activity, understand how visitors use our services, and improve performance.

A deletion or withdrawal request submitted to Reentry Care may not automatically delete information independently maintained by a licensed insurance agent or agency that previously received your information. You may need to submit a separate request directly to that partner.

Children’s Privacy
Reentry Care’s services are intended for adults and are not directed to children under 18. We do not knowingly collect personal information directly from children under 18 through our enrollment-assistance forms.

Changes to This Notice
We may update this Consent and Privacy Notice periodically. The updated notice will be posted with a revised effective date.

Material changes will apply prospectively unless otherwise permitted by law. When required, we will provide additional notice or obtain additional consent.
